Where can I request a vital record?
The Tennessee Health Department’s Office of Vital Records reviews, registers, amends, issues and maintains the original certificates of births, deaths, marriages, and divorces that occur in Tennessee in accordance with Tennessee Code Annotated.

Prohibits entities from denying an individual any service, financial aid, or other benefit because of race, color or national origin.

What is a Public Record?
“Public record or records’ or ‘state record or records’ means all documents, papers, letters, maps, books, photographs, microfilms, electronic data processing files and output, films, sound recordings, or other material, regardless of physical form or characteristics made or received pursuant to law or ordinance or in connection with the transaction of official business by any governmental agency.” T.C.A. § 10-7-301(6)
The test for determining whether a record is public is “whether it was made or received pursuant to law or ordinance or in connection with the transaction of official business by any governmental agency.” Griffin v. City of Knoxville, 821 S.W. 2d 921, 924 (Tenn. 1991).
How do I change my business address?
You can file form SS-4800 to change the mailing address by mail or in person. A change in Registered Agent or principal address requires filing Articles of Amendment and paying the statutory $20 filing fee. Forms can be found on our the Forms & Fees page for mail and in person filings.
What happens if I file a Title III (HAVA) complaint?
Complaints must be filed on a Title III HAVA complaint form. Complaint forms may be found at your local county election office, the State Coordinator of Elections office or online.
All complaints must be in writing, notarized, signed, and sworn to by the complainant. The complaint may be filed with either the Coordinator of Elections or the local county election office.
Once a formal complaint is filed, the Coordinator of Elections or designee will decide the resolution of the complaint.
A complainant may request a hearing on the record.
The Coordinator of Elections or designee will gather information which may include written responses from the parties. Once all the information is gathered, the Coordinator of Elections or designee will determine whether or not there is a violation of Title III.
If it is determined that a violation of Title III has occurred, the State shall provide an appropriate remedy in accordance with Title III.
If it is determined that no violation of Title III has occurred, the State shall publish the dismissed complaints of the procedures.
A final determination must be made within 90 days of the complaint being filed. Only the complainant may agree to extend this period of time.
If a final determination is not made within 90 days and no extension is granted, the complaint shall be resolved within 60 days through alternative dispute resolution. All previous proceedings, including the record and any correspondence, shall be forwarded to an administrative law judge for alternative dispute resolution.
What are the hours of work?
Normally poll workers report to work one hour before polls open and leave one hour after the poll closes. Check with your county election commission for more information.
What are the qualifications to be a poll worker?
To be a poll worker, a person must:
Be at least 16 years old
Be a registered voter in the county if 18 or older
Be able to read and write in the English language
Not be supervised by a county or municipal elected official on the ballot
Not be a candidate or close relative of a candidate. Close relatives are defined as the spouse, parent, father-in-law, mother-in-law, child, son-in-law, daughter-in-law, grandparent, grandchild, brother, sister, brother-in-law, sister-in-law, uncle, aunt, nephew, or niece of a candidate on the ballot or a write-in candidate
Government Employees Who Can Serve as Poll Worker:
All City, County and Metro employees (unless working directly under the supervision of an elected official on the ballot)
State of Tennessee employees
Federal employees – consult your Human Resources Department to ensure eligibility
A poll worker shall also:
Have strong clerical skills
Be able to solve problems
Be an effective communicator
Who must comply?
State and local government: Agencies distributing federal assistance or entities distributing federal assistance to the state or local government entity.
Higher education: college, university, or other post-secondary institution
Local education agency or system of vocational education, or other school system
An entire corporation, partnership, or other private organization, or an entire sole proprietorship
The entire plant or private corporation or other organization which is a geographically separate facility to which federal financial assistance is extended.
What is Title VI?
Title VI of the 1964 Civil Rights Act says, ”No person in the United States shall, on the grounds of race, color or national origin, be excluded from participation in, be denied the benefit of, or be subjected to discrimination under any program or activity receiving federal financial assistance.” 42 U.S.C.§2000d
Does my complaint qualify as a Title III HAVA complaint?
Title IV of the Help America Vote Act requires each state to establish a state-based administrative complaint procedure. This procedure allows individuals to file a complaint if any person believes that a violation under Title III of the Help America Vote Act has occurred, is occurring or is about to occur.
A violation of Title III would include issues dealing with the following HAVA sections:
voting system standards
provisional voting
voter information requirements
statewide voter registration database and
accessibility for persons with disabilities
If your complaint does not fall within one of the above categories, you may still file a complaint with your local county election office, the State Coordinator of Elections office or online using a general complaint form.
How do I become a poll worker?
If you are interested in becoming a poll worker, apply by filling out an application form. Poll workers are appointed locally based on need.
What are the training requirements for poll workers?
County Election Commissions are required by state law to provide training. This training provides all the necessary information and knowledge to be a successful poll worker.
How do I get the books?
All special library materials--books, magazines and playback equipment--are circulated through the mail postage free. Patrons must borrow books each year to remain eligible for the service.
What does Title VI not do?
Does not apply to Federal assistance provided through insurance or guaranty contracts, (e.g. FHA loan insurance).
Does not apply to employment, except where employment practices result in discrimination against program beneficiaries or where the purpose of the Federal assistance is to provide employment.
Does not apply to direct benefit programs such as Social Security.
Does not apply only to contracts and set-aside programs.
How do I start the service?
You must first complete an application for service and send it to the Tennessee library. All states provide the service for their residents.
Are poll workers paid or volunteers?
Poll workers are compensated for working at polling places at a rate determined by the county election commission and are also compensated for attending any required training sessions. Poll workers may also choose to volunteer their services, but most are paid.
How do I know what titles are available?
Bimonthly magazines will tell you about newly released books and related library services: Talking Book Topics and Braille Book Review. You may also order general catalogs. You can always call the library to ask about authors and titles. The National Library Service catalog is searchable online.
Where will I be assigned?
Every effort is made to assign a poll workers to their neighborhood voting site. However, poll workers must be willing to be flexible and consider assignments at other sites in the county based on need.
Where do I send the Title III (HAVA) complaint form?
Special playback equipment is loaned to patrons who use the library service. Books and magazines in the TLABM collection are recorded at a slower speed than conventional recordings and the books require special players. Players are mailed to patrons postage free. The library repairs and replaces equipment as necessary. For patrons who wish to purchase their own equipment, a vendor list is available.
May state employees become poll workers?
Yes. State employees may become poll workers.
What is provided?
The library service loans recorded, large print and braille books and magazines, music scores in large print and braille and special playback equipment. The TLABM book collection of over 50,000 titles includes popular fiction and nonfiction, best sellers, classics, history, biographies, religious literature, children's books and books in foreign language. What is the Records Management Division?
The Records Management Division (RMD) was established by Tennessee Code Annotated 10-7-303 to assist state agencies in establishing systematic controls for the efficient use and sound preservation of state records. The Division serves as the primary records management agency for the state of Tennessee and provides professional consultative and analytical records management leadership to agencies. This guidance aids in the appropriate development, utilization, disposition, retention, and destruction of records.
The Records Management Division is further directed by the Public Records Commission (PRC) to serve as administrative liaison between state agencies and the PRC; to establish procedural guidelines for paper and electronic records oversight and retention; and to coordinate efforts supporting the state's Paperwork Reduction and Simplification Act of 1976 (TCA 4-25-101).
What is the Records Disposition Authorizations process?
Records Disposition Authorizations (RDAs) are record retention guides state agencies follow to maintain their records properly. The Records Management Division oversees the records management process. What is an Administrative Dissolution?
An Administrative dissolution occurs for the following reasons:
Failure to file an annual report
Being without a registered agent
A foreign entity fails to maintain its assumed name due to a name conflict
Returned payment by a financial institution
What is a Certificate of Existence?
A certificate of existence is a document issued by our office that certifies that an entity is active and in good standing.
